    CSU Immunization Form

    University policy requires residents to have the following immunizations for their protection prior to admittance to the Charleston Southern University Residence Halls. Required immunizations are MMR, Hepatitis B, Tetanus-Diptheria, and Varicella. The remaining vaccinations are highly recommended. This form must be in English and completed, dated, and signed by your Health Care Provider. Parental signatures are not accepted.

    Housing will not be assigned until the form is completed and uploaded to the portal.

    Enrollment Fee

    All new students are required to pay a $300 enrollment fee prior to receiving campus housing.
